Competition Terms and Conditions

1. The total prize pool is $1100.00. This consists of five prizes of an iPod nano 8GB with armband worth $220.00 each payable to the prize winners.

2. Information on how to enter and prizes, forms part of these terms and conditions of entry.

3. The promoter is Australian Unity Health Limited ABN 13 078 722 568. Authorized under ACT Permit No:TP 10/02519.1 , NSW Permit No:LTPS/10/05448

4. Employees and their immediate families of Australian Unity Health Limited of 114 Albert Road, South Melbourne, VIC, 3205 and any of its related bodies corporate are ineligible to enter this competition.

5. The competition runs from 12.01am, 01/09/2010 to 11.59pm 31/10/2010 in all states (“Competition Period”).

6. To be eligible for the prize draw a person must be over 18 years of age and register for Wellplan online member services on the Australian Unity Wellplan website.

7. All Eligible Customers will be automatically entered into the random prize draw. There is no fee for entry.

8. The random prize draw will be held on 05/11/2010 at 2.00pm at Australian Unity, 114 Albert Road, South Melbourne 3205.

9. The prize-winners will be notified by phone and in writing by 09/11/2010.

10. If the prize is not claimed by 05/02/2011 a second chance random draw will take place on this date at 2 pm at Australian Unity, 114 Albert Road, South Melbourne 3205.The prize-winners will be notified by phone and mail.

11. The prize is not transferable or refundable.

12. The Promoter may require the winner to provide proof of identity, proof of insurance purchase, proof of age and proof of residency at the nominated prize delivery address. Identification considered suitable for verification is at the Promoter’s discretion.

13. The Promoter’s decisions in relation to all aspects of this promotion are final, and the Promoter will not enter into any correspondence

14. To the extent permitted by law, the Promoter shall not be liable for any loss or damage whatsoever which is suffered (including but not limited to indirect or consequential loss) or for personal injury which is suffered or sustained by any entrant/winner as a result of participating in this promotion, or accepting any prize.

15. The prize will be delivered by Registered Post. Australian Unity cannot be held responsible for any damaged, undelivered, lost or stolen packs and no replacement will be provided. Allow six weeks for delivery of the prize pack.

16. The Promoter accepts no responsibility for any tax implications that may arise from winning the prize. Independent financial advice should be sought.

17. All entries will be the property of the Promoter. The information entrants provide will be used by the Promoter for the purpose of conducting this competition and communicating with entrants as to future offers, information and/or marketing promotions. The Promoter may disclose entrants’ personal information to its contractors and agents to assist in conducting this Promotion or communicating with entrants. The Promoter is bound by the National Privacy Principles in the Privacy Act 1988. The Promoter’s privacy policy is available on the Promoter’s website at www.australianunity.com.au. Entrants can request access to the personal information the Promoter holds about them by contacting the Promoter at the address set out above.
