Let us help you keep those visits regular. We’ll give you up to $150 per person per calendar year on recognised health checks performed by a doctor in private practice where you are unable to claim through Medicare.

Be sure to download a doctor health check form for your GP to complete during your visit. Then, when you’re ready to claim, simply return the tear-off section along with your medical bill and a completed claim form to Australian Unity. Keep the other part of the form with your personal health information for your own reference.
